This page has several forms for filling out for regrade requests, get exam accommodations such as extra time, cannot take an exam, too sick to do work in the class, feedback on the class or UTAs, etc.
You are required to take the exam with your section. We will not be able to let people take the exam with the other section due to space limitations.
If you get accommodations or believe you have a reason to consider rescheduling the exam, then fill out this form by Dec 4, 2015.
ACCOMODATIONS or RESCHEDULE FINAL EXAM FORM
If you get special accommodations for exams (for example extra time), then Prof. Rodger or Astrachan should have received an official letter about your accommodations for this class.
You must fill out this form ONLY ONCE to let us know that you get accommodations:

We will let you know if we did not recieve a letter about your accommodations.
You must ALSO fill out the Makeup exam form below for Exam 1 and Exam 2 and the Final so we can schedule your exam with accommodations.
There is no additional form to fill out for APT Quizzes. Those are timed, so you just take the amount of time with extra time that you are allowed.
If you have accommodations (such as extra time) or need to reschedule the exam because you are sick or you think is a valid reason, fill out the form below.
EXAM 2 Accommodations/Reschedule FORM
Please note that having other exams the same week or same day is not a valid excuse. It is likely that almost everyone in the class also has other exams the same week or same day.
You can request the regrading of an assignment, apt or lab. We do not regrade lecture participation. A note will be posted on piazza when new grades are available. Once grades are posted you have a week to request a regrade.
FORM FOR EXTENSION ON CLASS WORK
Fill out this form if you need an extension for work (assignment, labs, apts) in this course because you are too sick to do the work or some other reason that you warrant should be excused. If you are sick you must also fill out a STINF form. If you are not sick but have another reason you think should warrent an extension, then in addition to filling out this form, you must also email Prof. Rodger or Prof. Astrachan and cc your Dean with the reason why you need an extension.
There is a link under Sakai announcements to forms where you can give Prof. Rodger and Prof. Astrachan feedback on the course or on particular UTAs. The feedback can be anonymous or not.
